2023-2024 Vaccination Requirements

Children attending pre-K through 12th grade in New York State must receive all required doses of vaccines on the recommended schedule in order to attend or remain in school. This is true unless they have a valid medical exemption to immunization.

Below listed are New York State Immunization Requirements for School Entrance: 

Vaccines for Pre-K and School Attendance for Grades K-5

  • DTaP

  • Hepatitis Vaccine

  • Measles/Mumps/Rubella (MMR Vaccine)

  • Polio Vaccine

  • Varicella (Chickenpox) Vaccine

Additional Vaccines
for Middle and High School

  • Tdap: Grade 6/11 years old

  • Meningococcal: Grade 7

  • Meningococcal Booster
    Grade 12 on or after 16th birthday.

Additional Vaccines for Pre-K

  • Haemophilus influenzae type b conjugate vaccine (HiB)

  • Pneumococcal Conjugate vaccine (PCV)

Please be aware that if your child is in need of any of these vaccinations and the school has not received the required documentation within the first 14 days of  school, your child will not be allowed to attend.

School Vaccination Clinic

Ellis Family Health Center is hosting their annual Back-to-School Vaccination Clinic to help children get their school vaccinations in a timely manner. This is a walk-in service. No appointment is needed.

What You'll Need: Please bring a printed copy of any vaccine record and documentation from the school (to assist in identifying vaccination needs) and insurance card.
