Hometown Health Centers at School

Students who attend Hamilton Elementary School, Mont Pleasant Middle School and Schenectady High School can receive healthcare at Hometown Health Centers at School.

A school-based health center is a doctor's office located in the school or on school property that provides a full range of health care services so students can avoid health-related absences and get support to succeed in the classroom.  The centers located in our district,operated by Hometown Health Centers, serve students in three schools (Hamilton ES, Mont Pleasant MS and Schenectady HS) within two school locations (Mont Pleasant MS, Schenectady HS).

There is no cost to use the service.  Parents, however, do need to enroll their child by completing and returning the Parental Consent Form (located in the Document Container below)

Who can receive care at the center?
Students who attend Hamilton Elementary School and Mont Pleasant Middle School can be served at the Mont Pleasant Center.

Students who attend Schenectady High School can be served at the Schenectady High School Center.  Phone number is 518-370-1440 Ext. 4432.

What are the hours?
The hours are 8 a.m. - 4:30 p.m. on days when school is in session.

What is the cost?
There is no copay for any child seen at the center.

How do I enroll my child?
Please complete the enrollment form and return it to your child's school.
